Becoming your own Health Care Advocate: For Oregon youth ages 12-17

Are you ready for the changes in health care that happen when you turn 18? For example, do you:

  • Know the names and specialties of your doctors?
  • Have a medical ID card or health history form?
  • Know when and how to make an appointment if you need one?
  • Know about the medications you take and why you take them?

    • If you answered no, don’t worry, we can help you prepare. This interactive workshop will introduce you to what is coming and give you tools to help you be ready. You can manage your health care and be your own advocate. We can show you how.
